Phoenix Health Pty Ltd is collecting your personal and health information to create your Phoenix Health account and provide you with our health services. This information is collected in accordance with the Australian Privacy Principles (APPs) and the Victorian Health Privacy Principles (HPPs).

Why we collect this information: To provide you with personalised health optimisation services, facilitate appointments, communicate about your care, and comply with legal obligations.

What happens if you don't provide it: If you choose not to provide certain information, we may be unable to provide some or all of our health services, or the quality of service may be affected.

Who we may share it with: Your information may be shared with healthcare providers involved in your care (with your consent), secure technology service providers, and government bodies where required by law.

AI-assisted processing: Phoenix Health may use artificial intelligence to process your de-identified health data (e.g. pathology reports, lab results) to assist practitioners in extracting health metrics. All personal information is removed before AI processing, and AI output is always reviewed by a clinician before being saved. No data is used to train AI models (see our Privacy Policy, Section 7).

Overseas disclosure: Some data may be processed on servers outside Australia (see our Privacy Policy, Section 6 for details).
