Your 12-month package
Includes GP reviews through the year. Estimated Medicare rebates of $302.55 are based on eligible in-person GP consultations.
GP reviews at 3, 6 and 12 months, continuous glucose monitoring, three DEXA scans, exercise physiology and dietitian support.
- 1 × 45 min and 2 × 30 min GP consults at 3, 6 and 12 months
- 60 min strength and aerobic fitness assessment with VO₂ max test, plus a 30 min recovery session. After Foundational Plus, your included VO₂ test is scheduled around months 6–9 to review progress.
- 3 × 45 min exercise physiology; 2 dietitian sessions
- 14-day continuous glucose monitoring with a food diary
- DEXA at 3, 6 and 12 months; repeat blood panel at 12 months
- Weight medication management if clinically indicated (prescription costs extra)

Estimated Medicare rebates of $302.55 are calculated from 1 × 45-minute GP consultation at $128.35 each (MBS item 44), plus 2 × 30-minute GP consultations at $87.10 each (MBS item 36). This assumes eligible in-person consultations. Rebates apply once the consultations have been provided and depend on eligibility and applicable item requirements. Telehealth rebates may differ. No allied health rebates have been deducted from the estimated maximum cost.

MONTHS01–03
Understand your starting point
First exercise physiology and dietitian sessions, 14-day glucose monitoring, and a GP review and DEXA at month 3. After Foundational: fitness assessment with VO₂ max and recovery. After Foundational Plus: use your existing VO₂ results to set goals, with repeat testing around months 6–9.
MONTHS04–06
Review and adjust
Second exercise physiology and dietitian sessions. GP review and DEXA at month 6.
MONTHS07–09
Support your progress
Third exercise physiology session, scheduled around your goals. If you joined after Foundational Plus, your included repeat VO₂ assessment and recovery are scheduled around months 6–9.
MONTHS10–12
Review the year
Repeat blood panel, DEXA and GP review at month 12.
Your Phoenix app keeps shared results and recommendations together between appointments. Imaging and medication are considered only where clinically indicated; additional costs may apply.
Can I start with Foundational or Foundational Plus?
Yes. Complete either Foundational Health Assessment or Foundational Health Assessment Plus before joining. Your chosen assessment is charged separately from the package. Can the timing be personalised?
Yes. This calendar shows one way to distribute the included appointments. Your team will confirm dates and coordinate exercise physiology and nutrition appointments around your needs. It does not add appointments to the package.
How is this different from Momentum12?
This is a 12-month package following either Foundational Health Assessment or Foundational Health Assessment Plus. Momentum12 is a separate 12-week program with direct entry.
