Before your Baseline visit
The Baseline is a defined first appointment, rather than a commitment to a longer programme. It combines measurements with time to discuss what they mean for you. You can use that conversation to ask questions, understand the next steps being recommended and decide whether you want further assessment.
What should I do before I attend?
Complete the intake form and ask the team about preparation for your appointment. Let them know about previous results or reports you would like to discuss. The team will confirm what to bring and how the visit is arranged.
Is the Baseline a full health assessment?
The Baseline is an initial snapshot. The Foundational Health Assessment adds a more comprehensive medical, diagnostic and movement review. Your doctor will explain whether further assessment is appropriate for your circumstances.
How does the $350 credit work?
Book a Foundational Health Assessment or Phoenix Momentum12 within 30 days of your Baseline and your full $350 Baseline fee is credited once towards that next step. The Foundational Health Assessment is $990 in total, leaving $640 after the credit. Contact our team before booking so they can confirm and apply your eligible credit. Credits and terms
Do I have to choose a programme afterwards?
No. Your visit includes a summary and a recommended next step. You can discuss the recommendation and ask about alternatives and fees before deciding on any further care.
